    Advances in medical education and practice: role of massive open online courses

    Massive open online courses (MOOCs) are increasingly available in the area of health and medicine. These MOOCs are offered through various commercial and noncommercial online platforms. When offered through reputable institutions, they can provide valuable access to reliable information without the constraints of time, geographical location, or level of education. Most current courses appear introductory in nature. In its drive for quality health care, the National Academy of Medicine has prioritized a focus on known chronic care conditions. Many of these conditions are shared internationally. Among its initiatives, the academy encourages consumer and professional groups, patients, clinicians, health care organizations, and universities to work together to identify evidence-based care processes consistent with best practices, organize major prevention programs to target key associated health risk behaviors, and develop systems to measure and evaluate improvements in the provision of patient- and family-centered health care. Carefully designed and collaboratively developed MOOCs would appear a valuable resource to contribute to these initiatives. Such MOOCs can, 1) increase the health literacy of the public with regard to the prevention and treatment of known chronic care conditions, 2) provide ready access to continuing professional, and interprofessional, education, and 3) explore innovative teaching models for student learning focused on patient- and family-centered care. MOOCs would also appear helpful to facilitate effective communication among international communities of patients and clinicians, including student clinicians, with shared interests. Further, the accumulation of MOOC data through large-scale measurement and analysis, obtained nationally and internationally, has the potential to assist in greater understanding of the risk for diseases and their prevention, with this translating into medical education, and authentic, patient- and family-centered methods for student learning.     Informant Personality Is Associated With Ratings of Memory Problems in Older Adults

    Memory complaints are a key diagnostic criterion for dementia and Mild Cognitive Impairment. Rating scales can be used to capture information about individuals’ memory problems from informants such as family members. However, problems with scale reliability suggest that individual differences influence the ratings informants provide. This project tested whether informants’ neuroticism was associated with their ratings of an older adult’s memory. In an online study, 293 volunteers completed a Five Factor personality questionnaire and used two memory questionnaires to provide ratings of memory problems in an older individual they knew well. Rater neuroticism correlated positively with estimates of memory problems: more neurotic informants provided higher estimates of memory difficulties in the person they were rating. A second study replicated this finding with 786 volunteers and another widely used memory measure, the AD8. In both studies, exploratory analyses suggested the effect size was large enough to impact on clinical practice

    Interdisciplinary Perspectives on Restraint Use in Aged Care.

    Restraint use in Australian residential aged care has been highlighted by the media, and investigated by researchers, government and advocacy bodies. In 2018, the Royal Commission into Aged Care selected 'Restraint' as a key focus of inquiry. Subsequently, Federal legislation was passed to ensure restraint is only used in residential aged care services as the 'last resort'. To inform and develop Government educational resources, we conducted qualitative research to gain greater understanding of the experiences and attitudes of aged care stakeholders around restraint practice. Semi-structured interviews were held with 28 participants, comprising nurses, care staff, physicians, physiotherapists, pharmacists and relatives. Two focus groups were also conducted to ascertain the views of residential and community aged care senior management staff. Data were thematically analyzed using a pragmatic approach of inductive and deductive coding and theme development. Five themes were identified during the study: 1. Understanding of restraint; 2. Support for legislation; 3. Restraint-free environments are not possible; 4. Low-level restraint; 5. Restraint in the community is uncharted. Although most staff, health practitioners and relatives have a basic understanding of restraint, more education is needed at a conceptual level to enable them to identify and avoid restraint practice, particularly 'low-level' forms and chemical restraint. There was strong support for the new restraint regulations, but most interviewees admitted they were unsure what the legislation entailed. With regards to resources, stakeholders wanted recognition that there were times when restraint was necessary and advice on what to do in these situations, as opposed to unrealistic aspirations for restraint-free care. Stakeholders reported greater oversight of restraint in residential aged care but specified that community restraint use was largely unknown. Research is needed to investigate the extent and types of restraint practice in community aged care

    Put on a Smiley Face: Textspeak and Personality Perceptions

    With the emergence of web 2.0 there has been a dramatic surge in user-generated content. Although the Internet provides greater freedom in self-presentation, computer-mediated communication is characterised by a more relaxed attitude to grammar, spelling and punctuation. The language of the Internet, or textspeak, may be suitable for casual interactions but inappropriate in professional contexts. We tested participant perceptions of an author’s personality in two distinct contexts (formal vs. informal) and manipulated the written information under three levels of textspeak: none, low and high. Participants judged the author as less conscientious and less open, but more emotionally stable when textspeak was used, however context had no impact. Personality perceptions of textspeak users differ to those who write in standard English and this is likely to extend to informal impression management contexts (e.g. online dating). These findings also have a number of implications, for example in terms of screening applicants via social media

    Sparsest factor analysis for clustering variables: a matrix decomposition approach

    We propose a new procedure for sparse factor analysis (FA) such that each variable loads only one common factor. Thus, the loading matrix has a single nonzero element in each row and zeros elsewhere. Such a loading matrix is the sparsest possible for certain number of variables and common factors. For this reason, the proposed method is named sparsest FA (SSFA). It may also be called FA-based variable clustering, since the variables loading the same common factor can be classified into a cluster. In SSFA, all model parts of FA (common factors, their correlations, loadings, unique factors, and unique variances) are treated as fixed unknown parameter matrices and their least squares function is minimized through specific data matrix decomposition. A useful feature of the algorithm is that the matrix of common factor scores is re-parameterized using QR decomposition in order to efficiently estimate factor correlations. A simulation study shows that the proposed procedure can exactly identify the true sparsest models. Real data examples demonstrate the usefulness of the variable clustering performed by SSFA

    Congenital cystic eye with multiple dermal appendages: a case report

    BACKGROUND: A partial or complete failure in the involution of the primary optic vesicle resulting in the formation of a cyst is an extremely rare anomaly known as congenital cystic eye. The primary optic vesicle is formed but instead of the anterior part of the vesicle involuting to lie in apposition with the posterior part, a cyst persists at birth and replaces the eye. CASE PRESENTATION: We report a case of congenital cystic eye associated with multiple dermal appendages in a 1-day-old female child. This condition presented at birth as a large orbital mass in the left orbit that bulged forwards and stretched the eyelids. No globe or any other ocular structures were identified in the orbit. Multiple dermal appendages were present in the adjacent part of the face below the left orbit and on the upper part of the neck. CONCLUSIONS: Congenital cystic eye is an extremely rare condition and with only 28 previous cases reported in the literature.     Genuine Personality Recognition from Highly Constrained Face Images

    People are able to accurately estimate personality traits, merely on the basis of \u201cpassport\u201d-style neutral faces and, thus, cues must exist that allow for such estimation. However, up to date, there has been little progress in identifying the form and location of these cues. In this paper we address the problem of inferring true personality traits in highly constrained images using state of art machine learning techniques, in particular, deep networks and class activation maps analysis. The novelty of our work consists in that, differently from the vast majority of the current and past approaches (that refer to the problem of consensus personality rating prediction) we predict the genuine personality based on highly constrained images: the target\u2019s are self ratings on a validated personality inventory and we restrict to passport-like photos, in which so-called controllable cues are minimized. Our results show that self-reported personality traits can be accurately evaluated from facial features. A preliminar analysis on the features activation maps shows promising results for a deeper understanding on relevant facial cues for traits estimation

    The Precursors and Products of Justice Climates: Group Leader Antecedents and Employee Attitudinal Consequences

    Drawing on the organizational justice, organizational climate, leadership and personality, and social comparison theory literatures, we develop hypotheses about the effects of leader personality on the development of three types of justice climates (e.g., procedural, interpersonal, and informational), and the moderating effects of these climates on individual level justice- attitude relationships. Largely consistent with the theoretically-derived hypotheses, the results showed that leader (a) agreeableness was positively related to procedural, interpersonal and informational justice climates, (b) conscientiousness was positively related to a procedural justice climate, and (c) neuroticism was negatively related to all three types of justice climates. Further, consistent with social comparison theory, multilevel data analyses revealed that the relationship between individual justice perceptions and job attitudes (e.g., job satisfaction, commitment) was moderated by justice climate such that the relationships were stronger when justice climate was high
